Role of combined exercise stress echocardiography and cardiopulmonary exercise test in chronic thromboembolic disease

Abstract Background Chronic thromboembolic pulmonary disease (CTEPD) without hypertension could cause significant exercise limitations. However, interventional or surgical treatments for CTEPD with mild normal pressure are on controversy. Purpose We aimed to evaluate cardiopulmonary function through test (CPET) stress echocardiography and determine whether can explain limitations in patients mPAP <30mmHg. Methods Patients diagnosed as less than 30mmHg was derived from our center registry April 2014 October 2021.Transthoracic (TTE) performed at baseline (resting state) immediately after CPET. TTE parameters CPET were compared hemodynamic measured by right catheterization. Results Total 37 enrolled. Of these, Thirty-five had previously been CTEPH undergone PEA, BPA, both. Most of the complained dyspnea WHO functional class II III. Pulmonary vascular resistance (PVR) slightly higher (185.0±102.2 dyne sec cm–5). Also VO2max decreased (23.1±6.5 mL/kg/min). In correlation analysis, PVR rest, lower during exercise. Meanwhile basal ventricular (RV) normal, an increase RVSP notably observed (RVSP: pre-exercise 36.2±11.9, post-exercise 60.7±19.3, p value <0.001). Furthermore RV deteriorated (TAPSE: 16.1±4.8, 12.9±5.0, Conclusions PAP showed limited capacity induced hypertension.
